		EDUCATOR LICENSING: APPLICATION INFORMATION & FORMS: RENEWAL REPLACEMENT FORMS FOR 2008 (GENERAL FORMS) (Updated 1/17/08)     OVERVIEW       Each year the Office of Educator Licensing mails renewal packets to Vermont educators whose license is due for renewal in the upcoming year. If your license expires on June 30, 2008, this packet will be mailed to you in November 2007. This page provides downloadable versions of many of the forms included in the mailed packet for use by individuals who may have lost or misplaced some of their packet materials. NOTE: The Renewal Form is NOT included as a download as it is tailored to an individual's specific endorsement area.     RENEWAL INSTRUCTIONS & REPLACEMENT FORMS FOR 2008 (Updated 1/17/08)       The following instructions and forms are offered as PDF files. PDF forms must be downloaded to your computer, printed and then completed either by hand or on a typewriter:       Educator License Renewal Instructions (Updated 1/17/08) This document provides information on how to renew a license and instructions for completing each of the forms. Educator License Renewal Form This form is NOT AVAILABLE FOR DOWNLOAD. You must return the original form that you received in the mail. Do not send a copy! Drop an Endorsement Form Complete this form if you are dropping an endorsement. Disclosure Form All the questions MUST be answered. Please print your name and social security number on the top of the form; sign and date the form at the bottom. Applicant's Statement Regarding Child Support, Taxes and Unemployment Compensation All three sections of this form must be completed.
