EDUCATOR LICENSING:
SPEECH LANGUAGE PATHOLOGISTS (SLP) & AUDIOLOGISTS

New requirements for licensure of speech language pathologists and audiologists went into effect on October 1, 2004. At that time, the Department of Education's Office of Licensing and Professional Standards began to issue these new licenses.

This document provides information on the speech language pathologist and audiologist licenses including who must acquire these new licenses and the processes for doing so. Information is also provided about how to convert from an educator license to a clinical license. Applications and forms are provided below:
