EDUCATOR LICENSING: SPEECH LANGUAGE PATHOLOGISTS (SLP) & AUDIOLOGISTS (Updated 11/18/09)
New requirements for licensure of speech language pathologists and audiologists went into effect on October 1, 2004. At that time, the Department of Education's Office of Licensing and Professional Standards began to issue these new licenses.
Information on Applying for Vermont Licensure as an Audiologist or Speech Language Pathologist This document provides information on the speech language pathologist and audiologist licenses including who must acquire these new licenses and the processes for doing so. Information is also provided about how to convert from an educator license to a clinical license. Applications and forms are provided below:

New Disclosure Form (Updated 11/18/09) All the questions MUST be answered. Please print your name and social security number on the top of the form; sign and date the form at the bottom.
Applicant's Statement Regarding Child Support, Taxes and Unemployment Compensation All three sections of this form must be completed.
Return Receipt Complete this form to receive verification of receipt of your renewal application packet by the department.
