Application forms contained
in the School Construction Planning Guide are available below in two electronic formats.
These formats allow you to download the form(s) to your hard drive, open them in
your preferred word processing application, and fill in the required information.
Because all the forms below require and original signature, the forms may be completed
using a word processing program, but they may NOT be submitted via e-mail. The forms
must be printed, signed and mailed (along with any required documentation) to the
department. |

Letter of Intent to Apply
for Construction Aid(Updated 7/7/06)
This is the preliminary application that shows intent
to apply for construction aid and is sent in with exhibits required before the regular
construction aid application which is for new school buildings or major renovations/additions. |

This application is used for any projects that extend the useful life of a building
but do not increase the building's size or capacity. |

This application
is for school construction projects when the estimated cost is under $100,000 and
the project is necessary to address health and safety threats to students and employees
created by unanticipated circumstances or events. |

This is an application to be used if a school district is purchasing an existing
building for conversion to school use. |
