Home Study Forms

Full Online Enrollment Packet 

Create and submit all your Home Study forms (enrollment packet) electronically at one time. 

• We recommend submitting your forms online.

• Its easy and you can save your progress and return later. 

• A copy of your forms will be sent to the email provided on the form, as well as confirmation of submission.

Individual Forms 

• The following online forms are individual, as opposed to the packet above. If you are contacted by the Home Study Team and are missing a section, need to update one of the forms, or provide more information, please use the links below to complete and submit a single form online.

• Enrollment Form A

  • ​This Form must be submitted every year for each student enrolling in Home Study.

• Professional Evidence Reporting Form B 

• This form must only be filled out by a Licensed Professional (e.g. a pediatrician, speech/language pathologist, special educator, etc.)
• A report card from a Vermont public school or preschool can be submitted instead
• Alternative professional evidence may also be submitted
  • e.g. ​a special education evaluation from a licensed special educator, IEP, 504 Plan, physician or psychologist/psychiatrist screening/summary results

• Minimum Course of Study (MCOS) ​

  • Please see the MCOS Guidance Page for more info. 

• Adaptations for Minimum Course of Study 

  • ​Only required for students with a disability

• Instructor Enrollment Form C 

• Must be filled out if student will receive instruction from someone other than the parent
• Must be filled out by Instructor or school where class is taken
• Does not need to be completed for online schools

• Assurance of Educational Legal Rights

  • Required whenever only one parent has legal educational rights, or if there is a difference in last name. (Both parents with educational rights and the student must have the same last name to skip the Assurance.)

Families who choose not to submit electronically online have the option to print and complete the applicable forms listed below to enroll in the Vermont Home Study Program. Please note: we are no longer able to offer pdf fillable forms due to a licensing restriction.
